At The House of Gladness, LLC, we provide weekly individual sessions following an initial assessment ($95) wherein the music therapist assesses the holistic level of need and ability of the potential client.

Individual music lessons are based on interest in either piano, guitar, ukulele or vocal study.

MUSIC THERAPY ASSESSMENT
Music therapy assessments are an essential initial consultation administered by MT-BC to inquire and anticipate how to address the individual needs of the client using evidenced based scales to determine best strategies to ensure optimal quality of services.

In order to receive services through Sunshine Health, the form must be completed in its entirety by client's primary care provider. Once completed, provider must fax form WITH client's most recent clinical info (such as an after visit summary) AND prescription for music therapy services.
